CVS Real World Evidence (DEMO) Study

This survey-based study is looking at how COVID-19 impacts people’s daily life and regular activities. It will help us understand how to best support people as they recover from COVID-19. Those who qualify for the study can earn gift cards as compensation for their time and effort.


Those who may qualify:

Adults who have recently tested positive for COVID-19, and self-reported at least one symptom.

Step 1. Register on this site.

Step 2. Answer screener questionnaire to confirm eligibility to participate.

Step 3. Review and complete a consent form.

Step 4. Take the surveys.

Earn Reward (Gift) Cards

  • As thanks, participants will earn reward (gift) cards as compensation for various surveys completed during the study. Earn a $10 reward (gift) card for taking the first survey and earn up to $20 more in rewards for taking 2 additional surveys over six months. The reward (gift) cards may be used everywhere debit cards are accepted. Restrictions may apply; see www.mydashcard.com for more details.
